Job Description
The post holder will assist the Hepatology team in the maintenance and ongoing management of HCC (Hepatocellular carcinoma) Surveillance Patient database.
To work with the Service Manager, the administrative team, and clinicians to support the pathway of Hepatology patients across both University Hospital Lewisham and Queen Elizabeth Hospital.
Responsibilities
- Responsible for reviewing and validating HCC surveillance data to support timely surveillance and follow up.
- Track designated HCC Surveillance Patient throughout their pathway, escalating issues of concerns promptly and arranging next steps where appropriate.
- To work as a team providing cross cover for the other members of the Hepatology administrative team across the Trust.
- The post holder will work closely with the Service Manager, the administrative team, and clinicians to support the HCC Surveillance Patient pathway.
- To maintain and manage the database of HCC Surveillance Patients.
- To work with Service Management and Business Intelligence team to review and further develop an automated HCC Surveillance Patient database as required.
- To liaise with the Outpatient Booking Appointments Office to ensure all patients have a booked appointment, investigation and procedure.
- Record details of patients referred via other routes other than General Practitioners and add to the database. Report to the Service Management teams any deviations from process.
- To administer entry into the database according to guidelines and monitoring milestones.
- Undertake validations and corrections to the HCC Surveillance Patient pathways ensuring the pathway is kept up to date.
- Ensure data is entered in a timely manner, checking accuracy and detail at all times.
- Negotiate with all clinical and administrative personnel within the Trust to establish good working relationships in order to improve the coordination of the patient journey.
