Active Pathways is seeking a dedicated and passionate Head of Care to join the leadership team at Astley View. This is an exciting opportunity to take ownership of care quality across the service, ensuring the people we support receive safe, effective, responsive and compassionate care that is truly person-centred.
Working alongside the Registered Manager and wider leadership team, you will provide visible leadership to care colleagues, oversee care planning and medication governance, drive continuous improvement and champion outstanding outcomes for the people who live with us.
The Role
As Head of Care, you will:
- Lead the delivery of high-quality, person-centred care across the service.
- Maintain oversight of care plans, risk assessments and electronic care records to ensure accuracy and compliance.
- Lead medication management processes, audits and competency assessments.
- Analyse audits, incidents, complaints and safeguarding information to identify improvement opportunities.
- Support admissions, assessments, hospital discharges and multidisciplinary working.
- Coach, mentor and develop senior care and support colleagues.
- Promote a culture of accountability, learning and continuous improvement.
- Provide assurance that care practice is safe, effective and consistently embedded throughout the home.

About You
To be successful in this role, you'll have:
- A Level 3 Adult Social Care qualification or above.
- Significant experience in a senior care role within a residential, nursing or dementia care setting.
- Strong knowledge of person-centred care, safeguarding, medication management and the Mental Capacity Act.
- Experience leading, coaching and developing care teams.
- Confidence using electronic care planning and medication systems.
- Excellent communication, organisational and decision-making skills.
- A passion for delivering exceptional care and improving people's lived experiences.

A Level 5 qualification in Leadership and Management for Adult Care, or progress towards this, would be advantageous.
