Health and Employment Specialist
Salary: £34,000 – £36,000 per annum
Contract: Fixed Term Contract – minimum 12 months
Hours: Full Time
Locations:
- Milton Keynes / Luton – covering Milton Keynes and Luton
The Role
We are looking for two Health and Employment Specialists to provide personalised, health-led support to individuals facing complex barriers to employment and independence.
You will provide person-centred, strengths-based support, helping participants improve their health, wellbeing, confidence and independence while progressing towards their goals at a pace that is right for them.
Using the Social Model of Disability, you will identify barriers, build on individual strengths and develop practical support plans. You will work closely with participants, colleagues, Jobcentre Plus, healthcare professionals, community organisations and employers to provide coordinated support and help individuals move closer to sustainable employment.

Key Responsibilities

- Deliver personalised one-to-one coaching and support to participants.
- Complete holistic assessments and develop personalised Support Plans.
- Help participants identify and overcome health, social and practical barriers.
- Build positive, trusting relationships using person-centred and trauma-informed approaches.
- Support participants to access health services, community support, volunteering, learning and employment opportunities.
- Use coaching and behaviour-change techniques to encourage positive progress.
- Monitor participant progress and measure outcomes and distance travelled.
- Work collaboratively with healthcare professionals, Jobcentre Plus, community organisations and employment services.
- Identify and appropriately manage safeguarding concerns and risks.
- Maintain accurate case records, outcome measures and contractual documentation.
- Work effectively as part of a multidisciplinary team.
Skills and Experience
- Desirable: Degree-level qualification (Level 6 or above) in a relevant health, wellbeing or behavioural science discipline, such as Psychology, Public Health, Occupational Therapy, Physiotherapy, Nursing, Counselling or Sports Science.
- Experience supporting individuals with physical or mental health conditions, disabilities or complex barriers to progression.
- Experience delivering person-centred support, coaching, rehabilitation or behaviour-change interventions.
- Understanding of the Social Model of Disability, safeguarding, professional boundaries and risk management.
- Strong communication, assessment and relationship-building skills, with the ability to engage individuals who may be anxious, disengaged or lacking confidence.
- Experience maintaining accurate case records and outcome information.
- Knowledge or experience of trauma-informed practice, motivational interviewing or health coaching would be advantageous.
- Experience in employability, vocational rehabilitation, public health or DWP-funded services would be desirable.
- Experience working within multidisciplinary teams and alongside healthcare, statutory or community organisations would be beneficial.
