Kome Care Adult Care Worker Apprentice
Kome Care are looking for a new Adult Care Worker Apprentice. You will be studying toward your Adult Care Worker apprenticeship Level 2. You will be supporting their service users in the Swanley area.
Most of your apprenticeship is spent working. You’ll learn on the job by getting hands-on experience.
What you'll do at work
We are looking for compassionate, reliable, and dedicated Healthcare Assistants to join our growing team. If you are passionate about making a positive difference in people's lives and delivering high-quality, person-centred care, we would love to hear from you.
As a Healthcare Assistant, you will provide essential support to individuals in their own homes, supported living settings, or other community environments, helping them maintain their independence, dignity, and quality of life. You will work closely with registered nurses, care coordinators, and other healthcare professionals to deliver outstanding care tailored to each individual's needs.
Duties will include:
- Meeting patients and their families to inform them of the role of the Care Assistant; commencing the care plan; offering information regarding treatments, procedures, prescriptions, and continuing care
- Participating in interdisciplinary meetings to develop care plans in conjunction with other care providers; ensuring smooth care plan advancement; promoting peer support; educating colleagues on case management practises
- Supervising quality and delivery of care by carrying out patient rounds; documenting care and progress towards desired care outcome; identifying and correcting digression from the documented plan of care; reviewing progress with patients in conjunction with the principal care provider; working alongside external departments to agree and expedite tests, procedures, and consultations; reporting staff and performance concerns to the manager; maintaining communication with operation staff regarding deviation from the care plan or transfer/release plan.
- Measuring effectiveness of plan by evaluating the results of care with the interdisciplinary team; providing care improvement recommendations
- Act in accordance with the care guidelines and legal requirements; fostering treatment practices and standards of care that comply with the values of the hospital and nursing division; observance of governing regulations
- Follow policies and procedures to prevent the spread of diseases and illnesses amongst self, colleagues, and patients
- Maintaining patient confidentiality and respecting the rights of patients
- Creating and implementing organisation procedures to maintain quality standards
- Recording patient information in the patient care database as it becomes available; backing up data; validating findings and reports
- Continually improving industry and technical knowledge by attending professional workshops; reviewing trade publications; building professional networks; familiarising oneself with new practices

- Apply national standards, organisational policies and procedures, codes of conduct and ways of working that apply to own role.
- Follow national legislation, policies and guidance in adult social care settings, relating to health and safety within the scope of own role.
- Consider the human rights of individuals in adult social care settings, in line with organisational policies relating to equity, diversity and inclusion to support an open culture.
- Demonstrate a duty of candour, and duty of care acting in the best interest of individuals to ensure they do not come to harm.
- Apply a person-centred, co-produced approach when developing relationships with individuals to support their health and wellbeing.
- Assist with conducting risk assessments, which focus on positive risk taking, to ensure the safety of yourself and others.
- Apply the principles of positive and proactive care and support for those with lived experience to proactively avoid or reduce escalation, de-escalate, and manage conflict.
- Customise information, advice and guidance to individuals with or without capacity or their representative to make informed, independent choices and decisions.
- Adapt approach to care and support for individuals with lived experience to meet their current and evolving cognitive and physical needs and conditions.
- Recognise and respect how an individuals capacity determines their ability to make informed decisions about their needs.
- Identify and respond to signs and symptoms of changes in the physical and mental capacity, mental health and wellbeing of those with lived experience, and monitor, record and report changes.
- Use appropriate communication methods that are adapted to respond to the needs of individuals, stakeholders and family members in adult social care settings, including verbal, written, non-verbal and digital communication.
- Develop personal resilience and access support to maintain wellbeing of self.
- Use record keeping systems to maintain clear and accurate records of interactions with individuals, and others ensuring security of information and data.
- Apply methods of digital working and communication and new care technologies to support improvements in own work setting.
- Record learning opportunities in line with organisational policies that support lifelong learning and meet the relevant standards.
- Record, use feedback and supervision to improve own practice to identify and support the development of individual goals and career opportunities.
- Contribute and agree to the personal development plan demonstrating support required for new learning goals and reflection on actual practice.
- Monitors safe and effective administration of medicines in accordance with national and organisational policies and the limitations of own role.
