Role Overview

The Health and Wellbeing Coach provides personalised support to individuals to empower them to take control of their own health and wellbeing with a focus on lifestyle behaviour change, inclusive but not limited to the core areas of:

  • Healthy Eating and Nutrition
  • Smoking Cessation
  • Physical Activity
  • Weight Management
  • Alcohol consumption
  • NHS Health Checks (outreach)
  • Delivery of a Wellbeing Service

As a Health and Wellbeing Coach, you will facilitate people in establishing and attaining health-promoting goals, set by the person to change lifestyle-related behaviours, with the intent of reducing health risks, improving self-management of chronic conditions, and improving quality of life.

Key Responsibilities

  • Facilitate people in establishing and attaining health-promoting goals

Key Duties

  • Provide triage and assessment
  • Set agendas
  • Offer lifestyle behaviour change support
  • Provide problem solving
  • Use motivational interviewing
  • Set goals
  • Provide signposting

Service Delivery

  • Deliver multi-channel, personalised health coaching support
  • Use face-to-face, telephonic, and digital health coaching
  • Utilize multi-mix communications methods including in-app messaging, text, and email
  • Share helpful and supportive resources
  • Advocate the use of behaviour tracking tools
  • Appraise individuals and population caseloads daily
  • Use a strength-based approach

Outreach Community Engagement

  • Participate in screening services (e.g., NHS Health Checks)
  • Engage in community and stakeholder events
  • Attend external meetings and networks
  • Participate in roadshows and pop-ups

Performance and Compliance

  • Work in line with business and contract performance requirements
  • Implement Standard Operating Procedures (SOP)
  • Fulfill policies and quality management framework expectations
  • Participate in regular audits and performance observations
  • Commit to continuous professional development

Reporting and Data Collection

  • Ensure accurate reporting and data collection
  • Manage caseloads efficiently, effectively, and at scale
  • Support in population behaviour change as required
  • Complement and enhance the behaviour change journey
  • Offer comprehensive and coordinated supportive services
  • Ensure data audits accurately capture activity, productivity, utilisation, and outcomes

Essential Qualifications & Experience

  • Relevant health coaching qualification or an accredited health coaching skills programme
  • A minimum of six months of direct health coaching delivery
  • Experience in delivering behaviour change interventions for specific lifestyle related issues
  • Evidence of supporting vulnerable individuals through a change process
  • Experience of caseload management
  • Experience of inputting and processing data
  • Evidence of continuous professional development
  • Experience of working in a health, community, or social care environment

Desirable Qualifications & Experience

  • Experience of managing and supervising individuals and teams
  • Educated to degree level in a relevant subject
  • Experience of supporting people remotely / telephonically / digitally
  • Evidence of working with individuals with long term health conditions

Individual Competencies

Essential

  • Strong understanding of the social determinants of health
  • Strong understanding of population-based approaches including segmentation and risk stratification
  • Strong understanding of behaviour change principles and methodology
  • Excellent oral and written communication skills
  • Demonstrable core skills and competencies as set out in best practice standards
  • Good understanding of principles of confidentiality and safeguarding
  • Strong written and verbal communication skills
  • Strong track record of interpersonal relationships
  • Effective caseload management
  • Confident in the use of evolving digital technologies
  • Ability to manage time independently and effectively
  • Effective safe and sensitive data management
  • Excellent Microsoft Office skills
  • Commitment to personal development / training
  • Commitment to flexible working (including evenings and weekends)
  • Commitment to supporting overall team/contract performance
