Health and Wellbeing Practitioner: HMPs Usk and Prescoed
Location: Usk
Salary: A GBP 26,000 per annum

Are you passionate about helping people make positive changes in their lives? As a Health and Wellbeing Practitioner, you'll play a key role in supporting individuals in prison to improve their health and wellbeing, reduce harm and prepare for a successful transition back into the community.
What You'll Be Doing
- Manage your own caseload, building positive and supportive relationships with service users.
- Provide appropriate harm reduction advice, guidance and support.
- Complete comprehensive assessments and formulate individualised care plans.
- Work collaboratively with service users to ensure true co-production and that their goals and needs remain at the heart of their care.
- Deliver structured 1:1 interventions and support sessions.
- Facilitate engaging and effective group work sessions.
- Support individuals with release planning, helping them prepare for life beyond prison.
- Coordinate effective through-care and transitional support, ensuring individuals are connected with the right services when returning to the community.
- Maintain accurate, timely and high-quality records, ensuring data is captured effectively.
- Work towards agreed targets and outcomes, using data to demonstrate the impact and effectiveness of your work.

- Is passionate about health, wellbeing and supporting positive change.
- Can build trusting, respectful and professional relationships with people from a wide range of backgrounds.
- Has excellent communication and interpersonal skills.
- Is confident delivering both 1:1 and group-based interventions.
- Can assess individual needs and develop person-centred care plans.
- Understands the importance of harm reduction and supporting people to make informed choices.
- Can work collaboratively with service users and professionals to achieve positive outcomes.
- Is organised, proactive and able to manage a caseload effectively.
- Understands the importance of accurate record keeping, data quality and meeting targets.
- Is committed to helping people make meaningful and sustainable changes.

Please note this is a full time 37.5 hours weekly vacancy working Monday to Friday with one late evening worked a week and one Saturday a month. The role is based across HMP Usk and HMP Prescoed. Both sites are inaccessible by public transport so a car is required.
