Riverside Centre Mental Health, Hillingdon
We are seeking motivated and compassionate two (2) Band 6 Registered Mental Health Nurses to join our dedicated Health Based Place of Safety (HBPoS) team. This is a unique opportunity to work within a highly specialised service based at Riverside Centre Mental Health Service, Hillingdon, offering excellent transport links and access across London.
As a Band 6 Mental Health Nurse, you will play a pivotal role in delivering high-quality, person-centred care to individuals experiencing acute mental health crises. The successful candidates will oversee and coordinate nursing practice, ensuring care is delivered safely, effectively, and in accordance with the Nursing and Midwifery Council (NMC) Code of Professional Conduct, the Pan-London Section 136 Guidance, and relevant Trust policies and procedures.
You will be expected to place the needs of service users at the centre of all aspects of care delivery while providing clinical leadership and professional support to colleagues. Working closely with a multidisciplinary team, you will contribute to the assessment, management, and care planning of individuals detained under Section 136/135 of the Mental Health Act and others requiring care within a hospital-based place of safety.
Main Duties of the Job
The post holder is responsible for:
Assessment and Clinical Care
- Undertake comprehensive mental health, physical health, risk, and nursing assessments upon admission to the Health-Based Place of Safety.
- Monitor and evaluate service users' mental and physical health needs throughout their stay.
- Identify and respond appropriately to signs of deterioration, self-harm, suicide risk, substance misuse, withdrawal symptoms, or medical emergencies.
- Provide immediate clinical interventions and nursing care in accordance with evidence-based practice and organisational policies.
Risk Management and Safety
- Conduct dynamic risk assessments and develop appropriate risk management plans.
- Maintain a safe therapeutic environment for service users, staff, and visitors.
- Implement de-escalation techniques and least restrictive interventions in accordance with legal and professional standards.
- Participate in emergency responses, including rapid tranquilisation, resuscitation, and management of medical emergencies where required.

Essential criteria
- Registered on the NMC register, Mental Health
- Numeracy skills required for the safe administration of medicines

Skills and Knowledge
Essential criteria
- Able to communicate with the multi-disciplinary team, present cases, take note of actions, update others in the team and use handover effectively. Speaking face to face but also by telephone and writing letters and reports
- Maintaining accurate, timely, concise and legible clinical records both electronic and handwritten which adhere to the CNWL policies
- Able to talk to people about sensitive issues in a supportive manner but also challenge others assertively where necessary
- IT Skills: Able to maintain electronic patient records. Use Microsoft Word for letter and report writing
- Nursing Skills: You are expected to have the skills required to act as a professional, registered nurse and adhere to the NMC Code of Conduct
- Work well with other members of the team including communicating effectively, being kind and considerate, reliable and enthusiastic.
- Help others in the team achieve their objectives and deliver safe, effective care
- Mentor, teach and support others in the team

Previous Experience
Essential criteria
- You need previous experience of working in the community / mental health services either as a nurse, a student nurse or health care assistant.
