Job Summary/Job Purpose:
The post holder will help facilitate the smooth running of the day unit, which provides day services to individuals with dementia.
The post holder will work occasionally without direct supervision in the community, however, will have telephone access to an identified qualified nurse for advice and guidance if undertaking outreach work.
The post holder will promote and hold a positive attitude to dementia care and ensure that individual needs for privacy, respect, dignity, and confidentiality are maintained at all times.
The post holder carries out assigned tasks involving direct care to those who have memory difficulties as directed by the Qualified Nurse.
- To undertake interventions with people with memory-related problems sometimes working independently within the day care setting and occasionally within the person's own home if required.
Main duties of the job

To undertake identified allocated tasks in relation to the care environment, and in relation to the direct care of the client group as identified by the Qualified Nurse and in accordance with BCUHB Policy and Procedures.

To perform tasks within the plan of care as delegated by a qualified practitioner.

To do everything possible to ensure the comfort and consideration of the client and his/her family or friends, and to relate with kindness and sympathy to all concerned.

Overcoming barriers to understanding where there are communication difficulties or when a client has physical or mental disabilities.

Assist less able patients with their meals by cutting up food, opening packets, or feeding.

When necessary, launder personal clothes to ensure clean and comfortable clothing is readily available for patients to maintain their personal needs and dignity.

Undertake other tasks as required by the Qualified Nurse / Team Manager.

To report any perceived changes noted in the client’s condition directly to the qualified nurse.

Work in collaboration with Social Services Support staff.
The post holder must undertake specific job-related mandatory training as documented in the directorate training plan. Ensure any concerns are brought to the attention of the qualified nurse.

Support and orientate new staff in their induction to day support services in conjunction with the Day Service Nurse and in association with the Team Manager.

The ability to speak Welsh is essential for this post.

Qualifications and Knowledge
Essential criteria
- NVQ Level 3 Or working towards level 3 to be achieved within 18 months of appointment and with relevant experience in working with people with Dementia.
Experience
Essential Criteria
- Previous experience within the health care sector or specifically in a Mental Health care setting.
Desirable criteria
- Previous experience within dementia care.
Aptitude and Abilities
Essential criteria
- Understanding of risk management and health & Safety issues.
- Understanding of patient confidentiality.

Essential criteria
- Fitness and willingness to use de-escalation and where required, safe handling, breakaway techniques (as appropriate) subject to relevant training according to BCUHB Policy.
