Are you passionate about helping people make positive changes to their health and wellbeing?

The Doctors Ti Rakau is seeking a motivated and compassionate Health Coach to join our team and support patients on their journey towards better health outcomes. As a Health Coach, you will work alongside our GPs, nurses and Health Improvement Practitioner (HIP) as part of an integrated primary care team. You will help patients build the knowledge, skills and confidence they need to manage their own health and wellbeing, while connecting them with the services and support available within their community.

About the Role

This is a permanent part-time position, working approximately 24 hours per week. Days and hours can be discussed with the successful candidate.

Health Coaches work in partnership with patients and their family/whānau to:

  • Help them understand advice provided by the general practice team
  • Support them to identify and achieve their own health and wellbeing goals
  • Build confidence and skills to better manage their health
  • Connect them with appropriate health, wellbeing and community support services

As a core member of the practice team, you will provide support through face-to-face, telephone and video consultations. You may also facilitate group programmes, assist patients to navigate health and wellbeing services, and work collaboratively with the wider clinical team to improve engagement and outcomes for patients who may be harder to reach.

About You

We are looking for someone who genuinely enjoys working with people and is passionate about empowering others to improve their health and wellbeing.

The successful applicant will have:

  • Excellent communication and interpersonal skills
  • A positive, motivating and non-judgemental approach
  • The ability to connect with people from a wide range of backgrounds and cultures
  • An empathetic, caring and supportive nature
  • Good organisational and administration skills
  • The ability to work both independently and as part of a multidisciplinary team
  • Legal rights to work in New Zealand

Experience working in health, community, social services, wellbeing, fitness, nutrition or a related field would be advantageous, but is not essential.
