THE ROLE
The Health & Wellbeing Coordinator will be responsible for the coordination, delivery, development, monitoring, and evaluation of Nottingham Forest Foundation’s Health & Wellbeing programmes. Working closely with the Health & Wellbeing Manager, practitioners, assistants, and external partners, the role-holder will lead the successful delivery of community-based physical activity, health improvement, mental wellbeing, and social inclusion programmes across Nottinghamshire.
The role combines programme coordination, community engagement, partnership development, and frontline delivery to support the Trust's mission of improving physical health, mental wellbeing, social connectedness, and healthier lifestyles within communities experiencing the greatest need.
The Health & Wellbeing Coordinator will oversee key programmes including Healthy Goals and Resilient Reds, both of which supported and administered through the Premier League Foundation & PFA’s Community Fund, whilst also supporting other Health & Wellbeing initiatives such as Walking Football, Forest Forces and Tricky to Talk.
The role-holder will work with a wide range of participants, including teenagers, adults, older adults, and individuals facing health inequalities, helping them access support, set personal goals, increase physical activity levels, improve mental wellbeing, and build meaningful social connections.
The Coordinator will also be responsible for programme compliance, quality assurance, stakeholder engagement, staff support, monitoring and evaluation, and contributing to the development and sustainability of the Trust's Health & Wellbeing offer.
KEY RESPONSIBILITIES
- Lead on-the-ground delivery of both the Resilient Reds and Healthy Goals programme, enabling a blend of physical & mental wellbeing activity and interventions, trips, and events for communities across Nottingham City and County
- Engage in proactive partner and stakeholder engagement, including NHS and Public Health sector organisations to meet audience/participant need and strengthen referral pathways into Foundation led health and wellbeing programmes.
- Work with the Health & Wellbeing Manager to connect and engage programmes with statutory and non-statutory mental health support services (such as Wolf Pack Project, NHS Talking Therapies) to develop opportunities for partnerships and referral.
- Coordinate and oversee Healthy Goals physical activity sessions, ensuring venues in high-priority areas host interventions such as physical activity, mental wellbeing, and health lifestyles guidance.
- Support the coordinating and promotion of Resilient Reds Hubs, utilising community spaces to enable the reach and engagement of priority audiences
- Proactively liaise with NFF internal stakeholders including Finance to ensure budgetary planning and alignment of programme operations, and Marketing to communicate and showcase programme information/success and grow participation/engagement
- Proactively connect and engage with statutory and non-statutory third-sector service providers, developing opportunities for partnership working and for services to collaborate with Hubs and programmes, particularly when there is latent demand/need for additional provision
- Continuously and proactively with work other internal and external stakeholders to grow the programme, improve its outcomes, and attain its KPI’s
- Lead efforts to secure ongoing funding for health and wellbeing programme, including identifying opportunities, support bid writing, and cultivating relationships with funders
- Collate and organise a range of participant data through qualitative and quantitative measures, such as registers, surveys, case studies, and individual testimony from the PLF & PFA funded programmes.
- Collate and maintain up-to-date programme data across monitoring systems, aligning with parameters set by the PLF & PFA, including ensuring all reporting deadlines are met.
- Act as a positive role model and representative of NFF, ensuring the same standards of other NFF health and wellbeing programme delivery staff
- Support other Health & Wellbeing programmes – including the incorporation of unique skills/knowledge/experiences into delivery – and support other Foundation programme delivery where appropriate
- Uphold all Foundation policies, maintain professional boundaries, be committed to the highest safeguarding standards and report concerns in accordance with procedure
- Be a positive role model for NFF at all times, and champion its programmes and impact.

QUALIFICATIONS & EXPERIENCE
Essential
- Qualification in Health, Wellbeing, Community Development, Coaching, Sport, Public Health, Mental Health, or a related field.
- Lived experience of programmes that support health/wellbeing/mental health including providing advice, guidance, and goal setting.
- Activity delivery (e.g. UEFA C/Multisport) or working towards.
Desirable
- Qualification/working towards/experience in mentoring.
- Qualification/working towards/experience in MHFA.
Personal qualities
Bringing a diverse range of community skills, knowledge, and experiences, the role-holder must possess excellent proactive planning and organisation skills. The role-holder will be an excellent communicator, community-oriented, and people-focused, whilst also being confident at proactively forming collaborative relationships with partners (NHS, Public Sector, third sector), strengthening referral pathways, and coordinating community programmes/hubs.
The individual will leverage their proactive organisation skills combined with their understanding of supporting under-served or under-represented communities, to coordinate the programme from end-to-end, including planning, session delivery, data collection & reporting, and partnering with stakeholders, thereby strengthening the programme’s growth & impact against its KPIs, and ultimately securing ongoing funding.

- Strong flexibility and adaptability.
- Strong stakeholder engagement skills, including collaboration with local authority, NHS, public sector, third/voluntary sector to strengthen support/referral pathways.
- Excellent administrative and Microsoft Office skills.
- Strong data literacy, including collation, presentation, and reporting to demonstrate impact against KPI’s.
- A holistic understanding of health & wellbeing community programmes and their determinants of engagement and success.
- A strong community-orientation, with knowledge and experience of community delivery to affect positive outcomes.
- A continuous improvement and ‘big picture’ mindset, and ability to identify improvements for growth and target attainment.
- Knowledge and awareness of safeguarding best practice.

ADDITIONAL INFORMATION
- Completion of an Enhanced DBS & vetting checks including right-to-work and reference checks
- Completion of all role-specific NFF mandatory safeguarding checks
- Willingness to work unsociable hours, including evenings, weekends
- Driving license and access to own vehicle is preferred
