Community Nursing Teams

Community Nursing Teams provide personalised care to housebound patients aged 18+ with complex and long-term conditions. Our aim is to help patients remain safe and well in their chosen environment, preventing avoidable hospital admissions through proactive, needs-led care.

Working as part of a multidisciplinary team aligned with Primary Care Networks, you will support holistic assessments, care planning, and delivery in partnership with patients and their carers.

The service covers a wide range of interventions, including:

  • Wound care
  • Medication administration
  • Bladder and bowel care
  • Supporting individuals to manage their long-term conditions

You will contribute to high-quality, patient-centred care that focuses on maintaining independence and achieving the best possible health outcomes.

As a Healthcare Assistant, you will visit housebound patients in their own homes, delivering care in line with personalised care plans.

You will work largely independently, organising your daily visits efficiently while documenting care and escalating concerns to the wider team.

You will take part in daily handovers and support additional patient-related tasks as required. There are also opportunities to take on champion or link roles, contributing to the development of the wider team.

You will be supported to build your skills and confidence through training and development opportunities. The service operates 7 days a week, including bank holidays, with shifts typically between 08:00 and 18:00, although some flexibility may be available.

Kent Community Health NHS Foundation Trust is due to integrate with Medway Community Healthcare on 1 October 2026. Following integration and to align our community services to the new Neighbourhood Model of care delivery, there will be a period of consultation which may result in changes affecting some services, teams or roles.
