Community Health Care Assistant Opportunity

The Community District Nursing Service in Luton is offering a fantastic opportunity for highly motivated and enthusiastic Community Health Care Assistants to join our service. The successful candidates must be able to demonstrate a passion for supporting adults with a wide range of health care needs.

We believe in working together across specialities, trusts, and sectors to achieve the best patient outcomes.

Previous Experience As a Health Care Assistant Is Essential.

Holding a UK driving licence and having access to a car is essential to this role.

Our Community Nursing teams provide care to patients in the privacy of their own home. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals.

We believe in practitioner-led care that treats the whole person, not just the condition, enabling them to stay at home longer and leave hospital faster.

Responsibilities:

  • Undertake the delivery of patient care packages under the supervision of nursing staff.
  • Manage and prioritise workload.
  • Liaise with other health professionals, social services, GP’s, hospital staff, and other agencies involved in the care of each patient as required.
  • Accurately take and record telephone messages as required.
  • Undertake set nursing procedures following training and assessment, e.g., phlebotomy, blood pressure monitoring, blood glucose monitoring, preparing patients as necessary prior to procedure, using aseptic technique.
  • Driving skills, including in icy and snowy conditions.
  • Acquire core skills in basic physiotherapy, nursing care, and nursing techniques.
