Role Description
This full-time, on-site role in Rhyl covers Healthcare Assistant, Registered Nurse, Mental Health Nurse, and GP ANP positions, depending on qualification and experience. Day-to-day responsibilities include:
- Delivering safe, person-centered patient care
- Supporting daily living activities
- Administering or assisting with medications in line with clinical competence
- Monitoring vital signs and symptoms
- Implementing care plans
- Maintaining accurate clinical documentation
- Adhering to health and safety, infection control, and safeguarding policies

Team members will collaborate with multidisciplinary colleagues, communicate effectively with patients, families, and carers, and respond promptly to changes in patient condition. Depending on role level, duties may also include clinical assessment, care planning, and contributing to service improvement.

Qualifications
- Strong patient care skills, including delivering compassionate, person-centered support and monitoring patients’ physical and mental wellbeing.
- Competence in manual handling and mobility assistance, following safe moving and handling techniques and equipment use.
- Good understanding of medicine administration or support (within role scope), including adherence to prescribing guidelines, documentation, and safe storage.
- Solid knowledge of health & safety, infection prevention and control, and safeguarding procedures within healthcare settings.
- Excellent interpersonal skills, with the ability to communicate clearly, build rapport, and work effectively within a multidisciplinary team.
- Relevant healthcare qualifications: care certificate or NVQ in Health and Social Care for Healthcare Assistants; NMC registration for Registered Nurses and Mental Health Nurses; appropriate advanced practice qualification and registration for GP ANPs.
- Previous experience in care homes, hospital wards, or community healthcare settings is highly beneficial.
- Ability to work flexibly, manage time effectively, and maintain professional standards in a fast-paced clinical environment.
