We are looking to recruit a number of Healthy Lives Advisors to work as part of our Children and Young People’s Healthy Lives Team within Public Health. These roles will have a particular focus on children and families. Working to promote the strengths, skills and gifts of the young people accessing support, the postholder will work with individuals, families, communities, and other partners to achieve long-lasting change.

The successful candidate will provide social prescribing support for the most vulnerable young people in the community, including those with long-term conditions, behavioural risk, low motivation, and confidence. They will manage a caseload and ensure delivery of appropriate and high quality support for young people, families, and carers. They will work alongside other Healthy Lives Advisors, GP practices, other Shropshire council teams, schools and community organisations to enable an all-encompassing approach to personalised care and enable people to work out how best to use the community, health and care system.

This position is one of several vacancies, with roles available on a:
- Permanent (30 hours per week)
- Fixed-term contract (26 and 37 hours per week)
- Casual basis
