Hepatology Pathway Co-ordinator
An exciting opportunity has arisen for an enthusiastic, adaptable and motivated individual to join our Liver Team as a Hepatology Pathway Co-ordinator, supporting our Fibroscan Technicians in delivering our new GP Direct Referral Fibroscan Service.
We are seeking a highly organised, compassionate and proactive individual with experience of working within the NHS or healthcare pathways. The successful candidate will possess excellent communication and interpersonal skills, strong attention to detail, and the ability to effectively prioritise and manage a busy workload.
Responsibilities
In this key role, you will coordinate the patient journey from GP referral through to Fibroscan assessment, ensuring patients receive a seamless and efficient service. Responsibilities will include:
- Managing and processing referrals
- Coordinating and scheduling appointments
- Liaising with patients, GPs and the wider multidisciplinary team
- Supporting effective communication throughout the patient pathway
- Managing and uploading data
You will also provide comprehensive administrative support to the Liver Clinical Nurse Specialists (CNS) and Fibroscan Technicians.
This is an excellent opportunity to contribute to the development of an innovative service while supporting patients through an important stage of their healthcare journey.
- Co-ordinate the Fibroscan pathway, ensuring compliance with RTT targets, Trust policies, and booking procedures.
- Manage waiting lists, booking appointments, processing email referrals, preparing clinic lists, and sending patient correspondence in a timely manner.
- Maintain accurate patient records and upload Fibroscan data to relevant Trust IT systems and databases.
- Provide administrative support to the Liver CNS team.
- Liaise with healthcare professionals and external organisations to support seamless patient care as required.
- Act as the main point of contact for patients, consultants, the Liver CNS team, and other healthcare professionals, ensuring enquiries are dealt with professionally and efficiently.
- Monitor patient pathways, identify delays or potential problems and escalate concerns to the Liver CNS team.
- Maintain confidentiality and comply with Trust policies, Information Governance requirements, and mandatory training obligations.
