Home First Team Manager

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare or working on the Staff Bank within these organisations. Nursing and AHP vacancies are also open to Student Nurses and Newly Qualified Nurses/AHPs.

We are recruiting for a reliable and adaptable Home First Team Manager to join our professional service.

The Home First Team Manager is responsible for the effective day-to-day operation of the Home First service.

This includes leading a multidisciplinary team of assessors, community support workers, and administrative staff to ensure the safe and timely delivery of person-centred care that supports recovery and promotes independence at home.

Key Responsibilities Include

  • Operational Leadership: Oversee the planning and coordination of staff rotas, ensuring appropriate deployment of support workers and assessors across the locality to meet service demands.
  • Case and Workforce Management: Hold responsibility for the management of a defined caseload, ensuring timely assessments, support planning, and discharge coordination.
  • Recruitment and Development: Lead on recruitment, induction, and training of staff. Promote a culture of continuous learning by supporting team members' development through mentoring, regular supervision, and performance reviews.
  • Service Monitoring and Reporting: Maintain and improve local systems to monitor activity levels, outcomes, and referral trends.
  • Quality, Safety, and Safeguarding: Promote and maintain a safe working environment for service users and staff. Respond to and escalate safeguarding, medication incidents, complaints, and other concerns as required, ensuring appropriate documentation and learning follow-up.
  • Stakeholder Engagement: Work collaboratively with wider system partners including KEaH, community nursing, social care, GPs, and acute discharge teams to ensure seamless care pathways and positive outcomes for individuals.
  • Innovation and Improvement: Identify areas for service development and support the implementation of new models of care or workflows that improve efficiency and patient experience.

Kent Community Health NHS Foundation Trust is due to integrate with Medway Community Healthcare on 1 October 2026. Following integration and to align our community services to the new Neighbourhood Model of care delivery, there will be a period of consultation which may result in changes affecting some services, teams or roles.
