Doncaster Home Treatment Team
Band 5 Home Treatment Practitioner
37.5 hours – 08:00 to 21:30 365 days a year
An exciting opportunity has arisen for a band 5 nurse to join Doncaster intensive Home Treatment Team. We provide short-term intensive support in the community to individuals with acute mental health needs to prevent hospital admission.
We provide a service between 08.00 and 21.30 7 days per week and staff are expected to work shift patterns to cover this. Most team members work 3 long shifts, which helps promote a good work life balance.
The team works in partnership with third sector and peer organisations to deliver person centred crisis care, therefore transparency and openness is vital.

Purpose of the Role
- To provide a clinical service that is patient focussed, accessible and responsive on a 7-day a week basis.
- To triage all mental health referrals into the Team to determine the priority for service response and the appropriate clinical pathway.
- To provide and deliver highly skilled nursing care, including full mental health assessment, care planning and care delivery, biopsychosocial interventions, clinical risk assessment and management, and promoting recovery and inclusion.
- To recommend and refer, where treatment is indicated, to the most appropriate team/service.
- To work collaboratively with the crisis house to lead on care for admissions as a service.
- To facilitate early discharge from the inpatient wards where clinically indicated.
- To deliver home based treatment interventions for all patients, ensuring best practice is implemented.
