MENTOR Adult Day Health
MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Applicants Must Have
- 🪪 Valid Full Driver’s License held for at least 12 months
- 🚗 Reliable vehicle for traveling to individuals’ homes
- 🎓Valid and verifiable Homemaker Certificate
Join our team as a Homemaker and provide one-on-one support to individuals in their own homes. This role focuses primarily on household support, meal preparation, errands, companionship, and helping individuals maintain a safe, comfortable home and their independence.

Key Responsibilities
- Cleaning, laundry, organizing, and other household tasks
- Meal preparation and meal planning
- Grocery shopping, errands, and essential household needs
- Companionship and support with daily routines
- Limited assistance with daily living needs, as appropriate
- Document services using a mobile device
- Communicate concerns or changes to appropriate team members
Qualifications
- High school diploma or equivalent
- Must be at least 18 years old
- Valid Homemaker certificate required
- Valid Driver’s License held for at least 12 months and reliable vehicle required
- Must pass required background checks
- Dependable, compassionate, organized, and able to work independently
- Previous homemaking, caregiving, or similar experience is a plus
