HOPE Worker Recovery Navigator (Health, Outreach, Prevention and Engagement)
Location: Portland House, Shalesmoor, Sheffield, S3 8UG
Working Hours: 37 hours per week, Monday–Friday, with occasional weekend working as required to support events.
Contract Type: Permanent
Salary: £25,877.80 - £34,240.03 per annum
Please note: The salary listed reflects the full earning potential for this role. Starting salaries depend on experience and progression within the band.

About the Role
We’re looking for a compassionate and proactive HOPE (Health, Outreach, Prevention and Education) Worker to join our team, supporting individuals affected by substance misuse to reduce harm, improve their wellbeing, and work towards long-term recovery.
You’ll provide practical, person-centred support through a combination of caseload management, outreach, harm reduction, and brief interventions. You’ll engage with people in a variety of community settings, helping them access treatment and supporting them to make positive changes at a pace that works for them.

Working closely with Recovery Co-ordinators, pharmacies, community organisations, and other partner agencies, you’ll also support people experiencing complex needs, including homelessness, mental health challenges, and offending.
What You’ll Do
- Manage a caseload, completing assessments, risk assessments, and person-centred support plans.
- Deliver brief and extended interventions to encourage positive change and support recovery.
- Provide targeted outreach and engagement, including walk-in assessments, referrals, and triage activity.
- Provide harm reduction advice, including safer drug use, safer sex, and blood-borne viruses such as HIV and Hepatitis B and C.
- Support needle exchange services and work with pharmacies and communities to increase access to naloxone.
- Support individuals using alcohol, opiates, and non-opiate substances, including IPEDs, NPS, and over-the-counter drugs.
- Work collaboratively with partner agencies to provide intensive support to people experiencing complex and multiple needs.
To Succeed in This Role, You’ll Need:
- An NVQ Level 3 in Health & Social Care or equivalent, or a willingness to work towards this.
- Experience supporting vulnerable people, for example within substance misuse, mental health, or social care.
- Strong communication and relationship-building skills, with the ability to engage people who may find it difficult to access services.
- A compassionate, proactive, and person-centred approach to supporting people with a range of needs.
- Experience of outreach or community-based work and knowledge of harm reduction and recovery approaches would be advantageous.
- A driving licence and access to a vehicle is desirable.
