Pay Level: 5, £30,160 - £35,530 (fully inclusive of Local Weighting Allowance)
Contract Type: Permanent
Working Hours: 37 hours per week
Location: Observatory House/ Wexham Park Hospital (minimum 2 days per week in the office)
DBS requirement: Basic

What You’ll Be Doing
Are you an organised and proactive coordinator with a passion for improving outcomes for people leaving hospital?
We are seeking an enthusiastic and highly motivated Business Coordinator to join our Hospital Hub team. This is a key role supporting the coordination of hospital discharge pathways and ensuring individuals receive the right support at the right time to enable safe and timely discharge from hospital.
- Working closely with health and social care professionals, you will manage referral processes, coordinate assessments and service activity, monitor performance data, and act as a central point of contact for partner organisations.
- You will play a vital role in supporting patient flow, reducing delays, and contributing to the delivery of high-quality integrated discharge services.
The successful candidate will have excellent organisational and communication skills, strong attention to detail, and experience of managing data and administrative processes within a fast-paced environment. You will be confident working collaboratively with a range of stakeholders and committed to delivering excellent customer service.

Annual increments, where applicable, are usually payable on 1 April, subject to six months’ service and satisfactory performance, until the maximum spinal column point is reached.
