Landau Education and Supported Employment is a charitable organization that supports over 900 people each year across the West Midlands through tailored routes into employment, access to learning, and social enterprise initiatives. The charity works with individuals facing complex challenges such as bereavement, poverty, mental health issues, illness, and barriers to education or work, offering intensive support plans and one-to-one Individual Placement Support (IPS). Landau delivers accredited and non-accredited learning, voluntary and employment placements, and training programs in partnership with statutory organizations and European-funded initiatives. Guided by a vision of helping people realize their full potential, Landau is committed to accessibility, inclusion, and equality, empowering individuals to build confidence, skills, and economic independence while positively impacting local communities.
Role Description
The Hospital to Home Support Worker is a full-time, on-site role based in Hereford, focused on supporting individuals as they transition from hospital care back into their home and community. Day-to-day responsibilities include:

- Providing person-centered practical and emotional support
- Helping people understand and navigate care plans
- Liaising with healthcare professionals, social services, and community organizations to coordinate ongoing support
- Assessing individual needs, developing tailored support plans
- Assisting with access to education, training, employment opportunities, and community resources that promote recovery and independence
- Monitoring progress, maintaining accurate records
- Ensuring that safeguarding and risk management procedures are followed at all times
- Contributing to multi-disciplinary meetings
- Promoting Landau’s values of inclusion and empowerment
- Supporting individuals to build confidence and resilience as they settle back into their home environment

Qualifications
- Experience in support roles, such as Support Workers or similar positions, with a focus on person-centered care and community-based support
- Strong Interpersonal Skills and Communication abilities, including active listening, empathy, and clear written and verbal communication with individuals and professionals
- Knowledge of Safeguarding practices, risk assessment, and maintaining professional boundaries when working with vulnerable adults and young people
- Basic Technical Support skills, including use of digital tools for record-keeping, reporting, and accessing online resources and services
- Understanding of health and social care systems, and experience collaborating with hospitals, social services, or community organizations
- Ability to work independently, manage a caseload, and prioritize tasks in a fast-paced, community-based environment
- Relevant qualification in health and social care, social work, counseling, or a related field (or equivalent experience) is highly beneficial
