Customer First is looking for motivated and dedicated Independence and Wellbeing Facilitators to join our growing team.

Your role and responsibilities
As an Independence and Wellbeing Facilitator, you will play a key role in supporting customers to maintain their independence and wellbeing. You will identify the most appropriate pathway for each customer's journey, provide advice and guidance, and connect people with the support and services they need.
In this role, you will:
- provide advice and guidance to customers and carers about their rights, available options and sources of support, while assessing eligibility in line with current legislation and policy.
- work in line with Suffolk's Heart of Care approach, demonstrating a commitment to making a positive difference to the lives of Suffolk residents.
- champion the use of Assistive Technology and Digital Care solutions to promote independence and improve outcomes.
- apply a strengths-based approach and work within the Signs of Safety framework.
- manage your own workload effectively, prioritising tasks and meeting deadlines while contributing to team objectives.
- share knowledge and best practice with colleagues to support team learning and development.
- complete a range of assessments and referrals, including carers' assessments, digital technology referrals and packages of care.
You will need:

You will have:
- a strong understanding of customer service principles and experience delivering high-quality support.
- the ability to analyse information, solve problems and make informed decisions.
- excellent teamwork skills and the ability to work towards shared goals.
- knowledge of local services and providers, enabling you to connect people with the most appropriate support.
- strong communication and interpersonal skills, with the ability to engage confidently and sensitively with people who may be vulnerable, distressed or under pressure.
- the ability to build and maintain effective working relationships with colleagues and stakeholders.
- good IT skills and confidence using a range of digital systems and technology.
