Band 6 Community Mental Health Practitioners
We are seeking four enthusiastic and motivated Band 6 Community Mental Health Practitioners to join our newly established Intensive Outreach Service (IOT) supporting adults with complex mental health. This is an exciting opportunity to help shape a new service delivering intensive, recovery-focused interventions to individuals who require enhanced support in the community.
Role Overview
The post holder will work as a Care Coordinator within a multidisciplinary team, managing a defined caseload and providing comprehensive assessment, care coordination, risk management, and evidence-based interventions. Working closely with Psychiatrists, Psychologists, Nurses, Social Workers, Occupational Therapists, Primary Care, and voluntary sector partners.
Responsibilities
- Undertake holistic biopsychosocial assessments.
- Develop and review care and crisis plans.
- Coordinate support across multiple agencies.
- Provide assertive outreach to individuals who may be difficult to engage.
- Deliver interventions in a range of community settings, including service users' homes, supported accommodation, and community venues.
- Monitor mental and physical health, manage risk, support discharge planning, and help prevent unnecessary hospital admissions.
Using a recovery-oriented and trauma-informed approach, you will ensure interventions address the psychological, physical, social, cultural, and spiritual needs of service users.

Key Duties
- Act as Care Coordinator for a defined caseload of adults with complex mental health needs, providing assessment, care planning, care coordination, and evaluation of person-centred interventions.
- Undertake comprehensive biopsychosocial, risk, mental capacity, and safeguarding assessments, developing formulations that inform recovery-focused care.
- Develop, implement, and review care plans, crisis plans, Dialog+ interventions, and risk management plans in collaboration with service users, carers, and partner agencies.
- Provide intensive outreach support in a range of community settings to promote engagement, recovery, independence, and relapse prevention, reducing the need for hospital admission where appropriate.
- Deliver evidence-based interventions that address the physical, psychological, social, cultural, and spiritual needs of service users.
- Work collaboratively with psychiatrists, psychologists, nurses, occupational therapists, social workers, primary care services, and voluntary sector partners to ensure coordinated and effective care.
- Monitor mental and physical health needs, support medication management, evaluate outcomes, and facilitate safe discharge planning and community reintegration.
- Respond effectively to mental health crises, safeguarding concerns, and clinical risk.

Working Locations
In order to meet the needs of the Trust, you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that falls under Barnet, Camden, Enfield, Haringey, or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.
