If you’re a Registered Nurse with experience supporting people with complex health needs, and you want a flexible role where your clinical expertise can make a real difference, this could be the opportunity for you.
The Role:
You’ll provide clinical oversight across community-based complex care packages, working closely with clients, families, carers and multidisciplinary professionals. Earn up to £32.00 per hour on a part-time/ zero-hours contract where travel across Wales is required.
You will:
- Complete comprehensive client assessments and identify clinical needs and risks
- Develop, review and update person-centred care plans and risk assessments
- Monitor changes in clients’ conditions, treatments and care requirements
- Audit MAR charts and medication documentation, addressing discrepancies and escalating concerns where needed
- Provide clinical guidance and support to care teams
- Deliver clinical training and assess and sign off staff competencies
- Monitor the quality and safety of care within each package
- Ensure care meets organisational, safeguarding and regulatory requirements
- Maintain accurate clinical documentation and provide clear updates to the Head of Clinical
- Escalate clinical concerns, risks or safeguarding issues appropriately
- Help establish new care packages, including being willing to work shifts where required

Requirements:
- Current NMC registration
- Experience supporting people with complex health needs
- Experience in community or home care settings
- Strong knowledge of care planning, risk assessment and medication management
- Experience auditing clinical documentation, including MAR charts
- Experience delivering training and assessing clinical competencies
- Strong communication, organisation and clinical decision-making skills
- A commitment to safe, person-centred care
- You must live in the West of the country and be willing to travel across Wales
- You must be willing to travel to different packages and work shifts when setting up new packages
