Job Overview
An exciting opportunity has arisen within the Urgent and Emergency Care (UEC) pathway of the Mental Health Services at Leicestershire Partnership NHS Trust to become part of the Crisis Resolution Home Treatment (CRHT) team as a Band 4 Pharmacy Technician on a permanent basis.
Based at the Bradgate Unit/out in the community as required, your role will be to maintain a safe, efficient, and professional medicines management service responsive to the needs of its service users, in accordance with current legislation and guidance.
The CRHT team at the Bradgate Unit has a multi-disciplinary approach to patient management. Supported by the Lead Pharmacist, you will demonstrate and develop your professional accountability, innovative thinking and contribute to the clear vision for the CRHT team.
You will be required to work your weekly contracted hours between Monday to Friday as per the needs of the service and ad hoc weekends and bank holidays on a rotational basis.
We offer training and personal development within the role and at our Learning and Development centre to ensure you have the knowledge and skills to meet patient needs and complete your revalidation CPD requirements.
As well as having a BTEC in Pharmacy Science or Equivalent and being an Accredited Checking Technician, the successful candidate should have good computer skills, demonstrate they can work accurately, have excellent communication skills and be proactive. They need to be able to work as part of a team, work in an organized manner, and have a willingness to learn. The successful candidate will be registered with the GPhC.

Job Description Summary (Main Duties And Responsibilities)
- Identify new patients accepted by CRHT team, contacting them, GPs and / or other healthcare providers to collate an accurate medication history and documenting this in the patients record. If discrepancies are identified, these should be dealt with or escalated appropriately.
- Identify potential pharmaceutical issues that may arise during their care, discharge planning and work with multi-disciplinary team to resolve these.
- Work as part of the MDT and be the point of contact for staff when managing medication needs.
- Liaise with community pharmacies and GPs regarding CRT electronic prescription issues.
- Manage the notification of medication supply issues within CRT.
- Escalate complex clinical queries to the pharmacist.
