Role Overview
As part of the Home First discharge model, the Discharge to Assess (D2A) team helps people move safely from hospital to the most appropriate setting.
You'll oversee the delivery of a high-quality administrative service that directly supports patient flow and better outcomes for people across Kent. You'll lead and motivate a dedicated administration team, ensuring work is coordinated effectively and service standards remain high.
No two days are the same. You'll be responsible for line managing staff, conducting appraisals, supporting attendance and performance, developing and improving administrative processes, allocating workloads, analysing and collating data, and managing complex spreadsheets. You'll also work closely with health and social care professionals, provider services, and system partners.
We're looking for a proactive, organised, and people-focused leader who thrives in a fast-paced environment. If you're an excellent communicator, enjoy problem-solving, and are passionate about making a difference behind the scenes of patient care.
Working Pattern
This hybrid role is based at Maidstone or Tunbridge Wells Hospital, with opportunities to work from home.
- Additional Working Pattern: Monday - Friday 7.5 hours per day
Integration Notice
Kent Community Health NHS Foundation Trust is due to integrate with Medway Community Healthcare on 1 October 2026. Following integration and to align our community services to the new Neighbourhood Model of care delivery, there will be a period of consultation which may result in changes affecting some services, teams, or roles.
