We're building clinically safe, evidence-driven AI that expands access to trusted mental health support and we're looking for a Lead Full-Stack Software Engineer who wants their work to genuinely matter.
The Role
This is a high-ownership, high-impact engineering leadership position at the heart of Velora - our generative AI support programme for anxiety and depression that went live with its first partner in September 2026.
You'll lead full-stack development across the platform, TypeScript and React at the front, C#/.NET and TypeScript APIs behind them, on Azure and hold technical direction for the initiatives you lead. In a team of roughly 13 engineers, your work is visible, your decisions matter, and what you ship reaches real people managing real mental health challenges.
Expect to spend most of your week in the codebase. The rest making sure the engineers around you are pointed at the right problem.
Two things make this different from most lead engineering roles:
- It's a regulated healthcare platform, Velora processes protected health information and is positioned as a low-risk medical device. What you build must survive a clinical safety review and an audit on top of a code review.
- You own quality end to end there's no QA team to hand off to. Engineers here own their test suites and hold the bar even when delivery pressure argues otherwise.
What You'll Be Doing
- Own delivery end to end architecture, implementation, release, observability, incident response, and the consequences when it breaks. Once something is yours, it's yours through to production
- Set technical direction, designs, patterns, and trade-offs across ieso and partner engineers. You approve designs and review code without managing the people who wrote it
- Keep knowledge with ieso - design docs, decision records, and runbooks that outlive the conversation, so no critical path depends on one person
- Lead AI-first engineering in practice - Claude Code and agentic workflows are in daily use. You use them well and set the guardrails for reviewing generated code in a regulated codebase
- Bridge Product, Design, AI, and Clinical - turning clinical and product intent into scope an engineer can act on, and flagging early when scope, timeline, and the safety bar don't all fit together
- Elevate the engineers around you - mentoring, pairing, and code review for ieso and partner engineers alike, with line management as a natural next step as the team grows

You'll Thrive Here If You...
- Have set technical direction on substantial work — with other engineers depending on those calls, and you stayed long enough to see which were right
- Are genuinely full stack - strong TypeScript and React, strong C#/.NET, and comfortable owning a feature from the browser through the API to the database
- Have production cloud experience as the person accountable when it broke, Azure preferred, deep AWS or GCP is fine
- Are hands-on and current - you write code most days and treat testing as a first-class skill
- Have real fluency with AI-assisted and agentic development and earned opinions about where agents help and where they quietly create work
- Have a track record of taking ownership of systems someone else built and becoming the person others rely on
- Bring high standards and low ego you share opinions about decisions, then commit fully once they're made

Bonus Points For...
- Regulated-environment experience - HIPAA, SOC 2, or ISO 27001
- Background in digital health or clinical software
- LLM systems in production
- Familiarity with Next.js and Terraform
