Lead Nurse Role

The Lead Nurse will provide visible clinical and professional leadership of the integrated discharge team. The Lead Nurse post is a senior leadership role focused on patient flow, complex discharges, and delayed discharge reduction.

Responsibilities

  • Lead and manage the Hillingdon Hospital Integrated Discharge Team.
  • Deliver a robust integrated discharge model.
  • Ensure effective, person-centred and integrated pathways of care out of hospital are embedded through interdisciplinary ways of working and clear, well managed processes and protocols.

About the Trust

The Hillingdon Hospitals NHS Foundation Trust is the only acute Hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics, and maternity services. The Trust’s services at Mount Vernon Hospital include routine day surgery, delivered at a modern treatment centre, a minor injuries unit, and outpatient clinics.
