Job Overview
We are seeking an enthusiastic and compassionate Learning Disabilities and Autism (LDA) Keyworker to join the North East London LDA Keyworking Service. This is an exciting opportunity to be part of a growing service that supports children and young people with learning disabilities and/or autism who are at risk of admission to hospital, placement breakdown, or experiencing significant challenges within their communities.
The North East London LDA Keyworking Service works across health, social care, education, and voluntary sector organisations to ensure children, young people, and families receive coordinated, person-centred support. As a Keyworker, you will play a vital role in bringing services together, supporting families to navigate systems, and ensuring the voice of the child or young person remains at the centre of planning and decision-making.
We are looking for an individual with experience of supporting people with learning disabilities, autism, and complex needs, who is passionate about collaborative working and achieving positive outcomes. The successful candidate will work autonomously while being part of a supportive multidisciplinary team and will help shape personalised support plans that reduce the need for crisis interventions and promote stability within the community.
Main Duties of the Job
The successful candidate will manage a caseload of children and young people on the Dynamic Support Register (DSR) and act as a dedicated Keyworker to individuals and families. Working closely with health, social care, education, and voluntary sector partners, the post holder will ensure that support is coordinated, proactive, and focused on the outcomes that matter most to children, young people, and their families.
The role involves:
- Facilitating multi-agency working
- Supporting Care (Education) and Treatment Reviews (C(E)TRs)
- Monitoring risk
- Contributing to safeguarding processes
- Helping individuals access the right services at the right time
The post holder will also support service reporting requirements, maintain accurate records, and contribute to the ongoing development of high-quality, person-centred care.

This is a varied and rewarding role that requires excellent communication, organisational, and partnership-working skills, alongside a commitment to improving the experiences of children and young people with learning disabilities and autism.
Essential Criteria
- Degree-level qualification or equivalent experience.
- Evidence of continued professional development (CPD).
- Experience working with individuals with learning disabilities, autism, mental health needs, or behaviours that challenge.
- Knowledge of safeguarding, risk assessment, and person-centred practice.
- Experience of multi-agency working across health, social care, education, or voluntary sector services.
- Excellent communication, organisation, and relationship-building skills.
- Ability to manage a caseload and work autonomously within a multidisciplinary team.
- Understanding of the Dynamic Support Register (DSR), Keyworking model, or the needs of individuals with complex care and support needs.
- Basic IT skills and experience maintaining accurate records.
Desirable Criteria
- Professional registration as a Nurse, Allied Health Professional, or Social Worker.
- Master’s level study or equivalent experience.
- Experience working within health, social care, or public sector services.
- Knowledge of local services and support available to people with learning disabilities and autism.
- Ability to travel efficiently throughout the service area.

Probationary Period
This post will be subject to a probationary period.
High Cost Area Supplement Outer London (B&D, Redbridge, Havering, and Waltham Forest, CEME)
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part-time).
