Join our dedicated Learning Disability Care Hub and help improve the health, wellbeing and life opportunities of people with learning disabilities across our communities.
As a Band 5 Community Learning Disability Nurse, you will work as part of a supportive multidisciplinary team, delivering specialist pathways, evidence-based interventions and person-centred care to individuals with learning disabilities and complex needs
- Managing your own caseload within the community.
- Undertaking specialist health and behavioural pathways, assessments.
- Supporting individuals in a range of settings, including their own homes.
- Promoting health, wellbeing and reducing health inequalities for people with learning disabilities.
- Working closely with families, carers, primary care, social care and partner agencies.
- Contributing to specialist pathways including Positive Behaviour Support, Physical Health, Autism, Epilepsy, Dysphagia and Dementia.
