Leaving Care Social Worker

🕒 Full Time | 12-Month Contract

Are you passionate about supporting young people as they transition into adulthood? Hackney Council is seeking an experienced and committed Leaving Care Social Worker to join our dedicated Leaving Care Team on a 12-month contract.

This is an excellent opportunity to work with care-experienced young people aged 16-25, helping them build independence, access opportunities, and achieve positive long-term outcomes. You'll join a forward-thinking local authority with a strong commitment to relationship-based practice, anti-racist practice, and delivering exceptional services to children, young people and their families.

The Role

As a Leaving Care Social Worker, you will:

  • Manage a caseload of young people leaving care, providing high-quality assessment, planning and intervention.
  • Develop Pathway Plans that reflect the individual needs, aspirations and strengths of young people.
  • Build positive and trusting relationships with young people, ensuring their voices, wishes and feelings are heard and acted upon.
  • Work collaboratively with families, housing providers, education settings, health services and partner agencies to achieve positive outcomes.
  • Assess and manage risk while promoting independence and resilience.
  • Prepare and present clear, analytical reports and maintain accurate case records.
  • Ensure statutory responsibilities, visits, reviews and performance targets are met within required timescales.

About You

To be successful in this role, you will have:

  • A recognised Social Work qualification.
  • Social Work England registration.
  • Experience working with children, young people and families within a statutory setting.
  • Strong assessment, risk management and care planning skills.
  • Excellent written and verbal communication skills.
  • The ability to manage a varied workload and work effectively in a fast-paced environment.
  • A commitment to anti-racist practice, equality, diversity and inclusion.
