What you'll do at work
To provide support with aspects of daily living such as person-centred personal, practical social and emotional support as detailed in the care plan. Promoting independence, dignity and encouraging people to do as much as they can to maintain their abilities. Provide emotional support and social contact to reduce isolation and maintain quality of life.
Main Duties
- To read the care plan, risk assessments and protocols for the individual person before providing support
- To log in and out of the person’s homes using the Carefree go app and ensure that all EMAR charts are signed if relevant and care notes are completed
- To always follow the policies and procedures of Portfolio Homecare
- To portray a professional image and represent the company in line with the Company’s high standards at all times
- To provide personal care: assisting with washing, bathing or showering, shaving, oral hygiene, dressing and toileting including continence support
- Helping with moving and handling, using equipment safely, observing changes in health, and reporting concerns immediately to managers and professionals
- Preparing meals and drinks, support with eating and drinking, monitoring intake where required
- Medication: Prompting, assisting or administering medicines in line with care plan and training. Following company policy and recording accurately on the EMAR chart on the app
- Light domestic work such as laundry, shopping and helping to maintain a clean and safe environment
- Completing visit records on the app and communicating effectively with colleagues, families and other professionals
- To report any concerns immediately
- To complete mandatory training and attend training sessions to ensure that you have the knowledge and skills to support the people in their own homes
- To provide social support such as talking and listening to people who use our services, helping them maintain contact with family and friends
- Enable people who use our services to maintain contacts in potentially isolating situations
- To report all changes in the health and social circumstances of people who use our services to a manager as soon as practical
- To attend supervisions, appraisals and team meetings when required
- To read policies and procedures and to inform the trainer if you do not understand
- To work as part of a team with colleagues and work closely with other agencies and professionals involved in the care of people who use our services, for example District Nurses
- To plan your annual leave and to request a minimum of a month in advance for a week or more of leave. Maximum two week at a time. Minimum of a week’s notice must be given for a day off
- Please remember annual leave is granted on a first come basis so if there are other colleagues off you will be given alternative dates
- Take all annual leave within the leave year April 1st to March 31st as it will not be carried over
- To ensure that your care is road worthy has an in-date MOT, car insurance is business insurance and is in date
- Ensure that you wear your uniform whilst working and have your ID badge on your person
- Ensure that you maintain the persons confidentiality and that your mobile phone is locked when not in use, that you ensure if you are talking on a telephone about a person there is no one near who can hear
- Ensure the safety of the person by checking windows are closed, doors are not left open and key safe numbers are jumbled without anyone seeing

What you'll learn
Course contents
- Apply national standards, organisational policies and procedures, codes of conduct and ways of working that apply to own role.
- Follow national legislation, policies and guidance in adult social care settings, relating to health and safety within the scope of own role.
- Consider the human rights of individuals in adult social care settings, in line with organisational policies relating to equity, diversity and inclusion to support an open culture.
- Demonstrate a duty of candour, and duty of care acting in the best interest of individuals to ensure they do not come to harm.
- Apply a person-centred, co-produced approach when developing relationships with individuals to support their health and wellbeing.
- Assist with conducting risk assessments, which focus on positive risk taking, to ensure the safety of yourself and others.
- Apply the principles of positive and proactive care and support for those with lived experience to proactively avoid or reduce escalation, de-escalate, and manage conflict.
- Customise information, advice and guidance to individuals with or without capacity or their representative to make informed, independent choices and decisions.
- Adapt approach to care and support for individuals with lived experience to meet their current and evolving cognitive and physical needs and conditions.
- Recognise and respect how an individuals capacity determines their ability to make informed decisions about their needs.
- Identify and respond to signs and symptoms of changes in the physical and mental capacity, mental health and wellbeing of those with lived experience, and monitor, record and report changes.
- Use appropriate communication methods that are adapted to respond to the needs of individuals, stakeholders and family members in adult social care settings, including verbal, written, non-verbal and digital communication.
- Develop personal resilience and access support to maintain wellbeing of self.
- Use record keeping systems to maintain clear and accurate records of interactions with individuals, and others ensuring security of information and data.
- Apply methods of digital working and communication and new care technologies to support improvements in own work setting.
- Record learning opportunities in line with organisational policies that support lifelong learning and meet the relevant standards.
- Record, use feedback and supervision to improve own practice to identify and support the development of individual goals and career opportunities.
- Contribute and agree to the personal development plan demonstrating support required for new learning goals and reflection on actual practice.
- Monitors safe and effective administration of medicines in accordance with national and organisational policies and the limitations of own role.

Desirable qualifications
GCSE in:
- English (grade 4–9 (A*–C))
- Maths (grade 4–9 (A*–C))

Pre-employment checks. Please be aware that upon a successful offer of employment, the company completes digitalised right-to-work checks and DBS applications via an external provider - 'Matrix Screening'. The company also completes an internal online social media search, in line with Keeping Children Safe in Education guidance.
