Community Nursing Team
Location: Kincardine and Mearns District Nursing Teams
Hours: 2 x part time post 22hrs/week each (1 post in Inverbervie and 1 post in Laurencekirk)

Join Our Community Nursing Team
An exciting opportunity has arisen for motivated and enthusiastic Assistant Practitioners to join our Community Nursing Team. Working as part of a multidisciplinary team, you will play a vital role in supporting the delivery of high-quality, person-centred care to patients within their own homes and community settings.
This is an excellent opportunity for an experienced Healthcare Support Worker or Assistant Practitioner who is passionate about delivering compassionate care and improving patient outcomes.
The Role
As an Assistant Practitioner, you will work under the direction and supervision of Registered Nurses whilst exercising a degree of autonomy within your scope of practice. You will contribute to the assessment, planning, delivery and evaluation of care, supporting patients with a wide range of health needs.
Your Responsibilities Will Include
- Delivering delegated clinical interventions and treatments.
- Supporting patients with long-term conditions and rehabilitation goals.
- Undertaking clinical observations and monitoring patient progress.
- Assisting with wound care and other delegated nursing procedures following competency assessment.
- Promoting health, wellbeing and independence.
- Maintaining accurate patient records and documentation.
- Working collaboratively with colleagues, patients, carers and partner agencies.

- Holds a relevant qualification (SCQF Level 7 or equivalent).
- Has completed PEMAP training or willingness to complete this.
- Has experience working within a healthcare setting.
- Demonstrates excellent communication and interpersonal skills.
- Is able to work independently as well as part of a multidisciplinary team.
- Has a compassionate, caring and professional approach.
- Possesses good organisational and time management skills.
- Is committed to delivering high standards of patient care.
Community nursing experience would be advantageous but is not essential as full support and development will be provided.
