About the Service
The service provides care to children and young people who are looked after by the local authority from 0-17 years. The cohort is highly vulnerable and presents with complex safeguarding needs.
Role Overview
- Primary Location: Heart of Hounslow Centre for Health
- Additional Work: Outreach and health promotion within the community
- Availability: Minimum of two days per week, dependent on service requirements
- Working Hours: Standard hours are Monday to Friday, 9:00am–5:00pm with occasional later sessions
Requirements
- Registered Nurse with current NMC registration
- Qualified Specialist Community Public Health Nurse (Health Visitor or School Nurse)
- Experienced Children Looked After Nurse or Specialist Safeguarding Nurse with experience of working with children and young people

- Work collaboratively with social care, education, health professionals, carers, and other partner agencies
- Deliver effective, child-centred care and improve outcomes for children looked after
- Close links with Community Paediatrics, specialties, and therapy services both within local hospitals and in the community
