Mainstream Transition Officer

An exciting opportunity has arisen at Oaklands for a Mainstream Transition Officer to support a smooth, effective transition for students moving from secondary school into further education, particularly those with learning needs, EHCPs, or who require support at college.

The role focuses on early identification of needs, person-centred planning, and ensuring appropriate support is in place from the start of the student’s college journey.

Key Responsibilities

Transition Planning & Coordination

  • Consult with secondary schools, local authorities, parents/carers, and external agencies to gather information about students who are applying to attend courses at Oaklands College.
  • Attend Year 10 and Year 11 annual reviews (including EHCP reviews) to support transition planning.
  • Coordinate transition activities such as college tours, taster sessions, transition days, and welcome events.
  • Support the enrolment of students they have previously worked with, ensuring continuity of support and appropriate provision is in place.
  • Contribute to induction week planning to ensure appropriate support is in place for new starters.
  • Develop and implement individual transition plans for students with additional needs and liaise with curriculum teams to ensure that they are aware of how to meet student needs.
  • Liaise with the Managers and Heads in both specialist and mainstream provision at the college to ensure that information is shared appropriately.

Student Support & Engagement

  • Build relationships with prospective students to reduce anxiety and support readiness for college.
  • Provide tailored advice and guidance to students and families about support available in college.
  • Support learners during taster sessions, welcome days, enrolment, and induction week to ensure a positive and inclusive experience.
  • Provide continuity of support for students they have previously worked with during key transition points.
  • Undertake structured check-ins with students at 1, 3, and 6 weeks to monitor progress, identify support needs, and promote a successful transition.
  • Support vulnerable learners, including those with SEND, social, emotional, and mental health needs or other students who may need support at college.
  • Promote independence and self-advocacy in students.

Information Sharing & Documentation

  • Collect, review, and share key student information (e.g., EHCPs, support needs, reports) with relevant college teams.
  • Ensure all transition documentation is accurate, up to date, and compliant with data protection regulations.
  • Maintain detailed records of student interactions and transition activities.

Multi-Agency Working

  • Work collaboratively with secondary school SENCOs, local authority SEND teams, careers advisors, and external support services to build up a network of professionals around students progressing into college.
  • Function as a key point of contact for transition-related communication.

Internal Collaboration

  • Consult with curriculum staff, ALS teams, and safeguarding teams to ensure appropriate support is in place.
  • Contribute to planning for in-class and out-of-class support.
  • Support the development of inclusive practice within the college.

Safeguarding & Compliance

  • Safeguard and promote the welfare of young people and vulnerable adults.
  • Follow all college safeguarding policies and procedures.
  • Ensure all work complies with SEND legislation and statutory requirements.
  • Ensure that belonging and inclusion are embedded into everything that you do.

Essential Criteria:

  • Experience working with young people, particularly those with SEND or additional needs.
  • Knowledge of EHCP processes and SEND legislation.
  • Strong communication and people skills.
  • Ability to build positive relationships with students, families, and professionals.
  • Excellent organisational and record-keeping skills.
  • Experience in a college or secondary school environment.

Desirable Criteria:

  • Understanding of transition pathways into further education.
  • Experience attending or contributing to annual reviews.
  • Relevant qualification in education, youth work, or SEND.
