Job Overview

To support the delivery and development of high-quality memory assessment and dementia care services within the Central (Conwy/Denbighshire) Memory Assessment Service. To act as an autonomous practitioner, managing a caseload of individuals undergoing assessment, diagnosis and ongoing support for cognitive impairment and dementia. To work collaboratively with multidisciplinary teams, service users, carers and partner agencies to ensure person-centred care is delivered in accordance with Health Board policies, professional standards and the NMC Code. To contribute to service improvement and the development of colleagues through clinical supervision, mentorship, education and the application of evidence-based practice, research and emerging developments in dementia care.

Main Duties of the Job

The successful candidate will be a part of the Memory Assessment Service where they will work with those who are experiencing cognitive changes and have concerns regarding Dementia. The successful candidate will undertake responsibility of the Memory Assessment Service in the absence of the manager. They will be required to divide their time equally between a clinic and community care setting due to the large rural geographical area with limited transport links and problematic mobile phone reception. It is necessary to work autonomously and conduct patient home visits. To have the ability to make complex decisions unsupervised due to working in isolated areas. The successful candidate is expected to participate as a full member of the multi-disciplinary team, working with people who present with possible organic mental health problems. Taking responsibility for a caseload of patients and initial comprehensive assessments, delivering diagnosis and formulating treatment plans and initial review.
