This post is fixed term / secondment for 12 months due to Maternity Cover

The post holder will undertake responsibility of a cohort of patients on the MAS Assessment and Diagnostic Pathway.
The post holder may be required to divide their time equally between a clinic and community setting due to the large rural geographical area with limited transport links and problematic mobile phone reception.
It is necessary to work autonomously and conduct patient home visits. To have the ability to make complex decisions unsupervised due to working in isolated areas.
The post holder is expected to participate as a full member of the multidisciplinary team, working with people who present with possible organic mental health problems. Taking responsibility for a case load of patients and initial comprehensive assessments, delivering diagnosis and formulating treatment plans together with robust risk assessments within Primary Care Older Persons Mental Health Services (West Area)
Main duties of the job
An opportunity has arisen for a Mental Health Nurse to join us in the Gwynedd and Anglesey Memory Assessment Service (MAS). This is a long established accredited multi-disciplinary memory assessment service working with the RCPSych MSNAP Standards and the All Wales Pathway of Care Dementia Standards

We will provide you with the opportunity to develop transferrable skills - to become specialised in the assessment process by further developing your assessment techniques whilst carrying out in-depth first line assessments; collecting and collating information in an appropriate format to present to your MDT colleagues; sharing and discussing sensitive information as well as participating in the diagnostic process; providing knowledge and support during those early days of diagnosis as well as being involved with initial therapeutic group/ individual work. We want to support you in your career and give you opportunities to progress.
The role will require a mix of nursing skills from a range of professional specialist areas – Primary Care, Liaison, Adult Mental Health and Older Person’s Mental Health. As the Band 6 you will be expected to lead an area of the MAS in the absence of the manager. This is a unique opportunity to join our team during these interesting times of change.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
