Crisis Resolution Home Treatment Team Practitioner

An opportunity has arisen for mental health practitioners looking to join a crisis resolution home treatment team, providing specialist mental health care to the population of Tameside and Glossop. The CRHTT work within the MDT including psychiatrists and psychologist to support residents with severe and enduring mental health conditions. We aim to provide outstanding care to the people of Tameside and Glossop and listen to improve, in response to the needs of the population we serve.

This is an exciting time for CRHTT. We are currently undergoing a transformation within our service. It is an opportunity to be part of our service development, with planned investment in our staff and role development.

The CRHTT work with people aged 18 and over with severe and enduring mental health needs and support those in crisis within their own homes as an alternative to hospital admission. The CMHT works closely with stakeholders, providing a multidisciplinary approach to the care provided.

Responsibilities

  • Provide holistic assessment, interventions, risk assessments, and care plans for service users referred to the team.
  • Assessment of healthcare needs.
  • Producing collaborative care plans.
  • Risk assessment and management.
  • Monitoring and evaluating care plans.
  • Liaising with the wider MDT.
  • Providing evidence-based therapeutic interventions.
  • Providing education advice, emotional, and psychological support.
  • To promote choice in service user involvement in care planning and risk management.
  • Ensure carer involvement in the service user's care.
  • To report concerns identified under Safeguarding adult procedures.
  • Maintaining accurate records.
