Crisis Resolution & Home Treatment Team (CRHT)
CRHT operates on a 24-hour, 7-day-a-week basis, providing intensive home support to service users as an alternative to hospital admission. The footprint covers Halton, Warrington, St Helens, and Knowsley.
About the Role
CRHT is looking to recruit Band 6 practitioners to join the team. The post holder will be expected to work on a rotational shift basis in accordance with service demands, which includes weekends and night shifts.
Responsibilities
- Assessment of Care Needs: The post holder will be responsible for the assessment of care needs and the development, implementation, and evaluation of programmes of care within the framework of Effective Care Co-ordination (ECC).
- Integrated Care: Operate as part of an integrated Home Treatment Service but be flexible in delivering care within a variety of environments.
- Complex Needs: The team will work to assess and provide treatment to service users with complex needs. Alternative options to admission will be considered where possible.
- Service User-Centered Care: The post holder will demonstrate an attitude which respects and values service users' choice, working alongside them and their carers to ensure they are at the center of care planning.
- 24hr Roster System: Participate in a 24-hour roster system 7 days a week, on a rotational basis as and when the service demands.
