This research aims to evaluate how helpful mentalisation based therapy (MBT) is for young people aged 10 to 17 years who have been referred to Forensic Child and Adolescent Mental Health Services (FCAMHS) in England due to aggressive or violent behaviours. The trial compares the new MBT support, which helps young people understand their own and others feelings and behaviours, to the usual support provided by FCAMHS. This study uses a randomized controlled trial design to see which support works better for these vulnerable young people. Participants will receive either the usual support from FCAMHS or the usual support plus MBT for six months. MBT involves meeting regularly with a mental health practitioner, with parents or carers able to join some sessions. Usual support may include advice, guidance, psychoeducation, or other mental health treatments like cognitive behavioral therapy (CBT). After six months, participants will complete questionnaires again, and some may take part in optional interviews to share their experiences. During the study, young people will complete several questionnaires at the start and after six months to measure aggression and other mental health outcomes, reported by both the young person, their parent/carer, and clinicians. The primary outcome measure is the Reactive and Proactive Aggression Questionnaire completed by the young person. The study will also monitor safety and progress through these assessments. Participation lasts for six months, with initial and follow-up evaluations to understand the therapy's impact.

Eligibility Criteria
You may qualify if you...
- Age 10-17 years at the time of referral
- Aggression and/or violence indicated at referral or assessment
- Participation would not interfere with any legal or statutory orders
- Child and parent/carer (for ages 10-15) able to consent and engage with study materials
- Living in the community with no immediate plans to move to hospital, residential, or secure settings
- For clinician caseload recruits: no direct contact or therapeutic support previously provided by FCAMHS
- Clinician consultations allowed only if no direct child contact

- No evidence of aggression or violence at referral or assessment
- FCAMHS offering only a one-off consultation or assessment, without ongoing input
- Insufficient English to engage with the intervention
- Child and/or parents/carers refuse to engage with FCAMHS
- No statutory service involved in supporting the child
- No statutory service will keep the child's case open for at least six months
- Currently involved in another psychological intervention research study

Screening
Duration - 2 to 4 weeks
Participants are screened for eligibility to participate in the trial.
1 visit (in-person)
Outpatient Treatment
Duration - 6 months
Participants receive mentalization based therapy or support as usual to address aggressive and violent behaviour.
Regular outpatient visits as part of therapy sessions
Follow-up
Duration - At 6 months post-baseline
Participants complete assessments to measure treatment impact on aggression and related outcomes.
