Are you passionate about improving outcomes for children and young people facing complex mental health challenges?
We are looking for an experienced and motivated Outreach Liaison Practitioner to join our dynamic Mindworks Hub team. This is a unique opportunity to work at the centre of an innovative, multi-agency service, supporting children, young people and families whose needs require intensive coordination, creative problem-solving, and strong professional leadership.
As an Outreach Liaison Practitioner, you will play a pivotal role in ensuring that children and young people receive the right support at the right time, particularly those whose needs have not been met through traditional pathways.
About The Role
You will hold a small, focused caseload of children and young people with complex and often longstanding needs, acting as the Lead Professional to coordinate care and strengthen support networks around them.
You Will Work With Young People Who
- Are at risk of placement breakdown or escalation
- Struggle to step down following extended interventions
- Find it difficult to engage with traditional therapeutic pathways due to systemic complexity
- Require a stronger, more coordinated professional network around them
- Have experienced multiple referrals with ongoing unmet needs for themselves and their families

Travel across Surrey and home visits will be an essential part of the role.
