About the Role

We are currently looking for experienced B5 nurses to join our Neighbourhood Nursing teams across Lambeth and Southwark areas that has extensive experience in a community setting with the relevant skills outlined in the job specification.

We are one of only two community teams in the UK to receive an outstanding status by CQC. Come and join us, and become a part of the amazing work we do!

Eligibility

This advert is open to all applicants with an active Nursing and Midwifery Council Personal Identification number (PIN) (this refers to an NMC Pin identification that allows you to work as a registered nurse/midwife in the UK. This does not include the Personal reference number (PRN) when you apply pre-OSCE application).

Role Details

The post-holder will be a member of the Neighbourhood Nursing (District Nursing) Team and will participate in the clinical case management of patients who have a wide variety of complex, high-risk and supported/self-care conditions. Working directly with the Team Leader, the post-holder will help ensure the provision of a high standard of nursing care and health promotion. The care will be provided in a range of settings including patients’ homes, residential homes, nurse led clinics and GP practices.

Responsibilities

  • Holistic assessment, planning, implementation and evaluation of a plan of care for patients with long term conditions, tissue viability, continence, palliative care, and promoting/facilitating patient independence.
  • Assists in the delivery of National Service Frameworks.
  • Providing public health and health promotion programmes to improve health and reduce inequalities.
  • Maintaining contemporaneous written and computerised records.
