Are you passionate about supporting people in their rehabilitation journey? Join our Integrated Stroke and Neurorehabilitation Service as a Community Neuro Rehab Assistant and make a real difference in patients’ lives.

What We Are Looking For

  • Strong written and verbal communication skills.
  • A proactive, compassionate approach to patient support.
  • Ability to work independently and as part of a multidisciplinary team.
  • Essential: Car driver with a valid UK driving licence and access to a car for business use.

What You Will Do In This Role

  • Deliver neuro-rehabilitation programmes in outpatient, home, and community settings under the guidance of qualified therapists.
  • Support Physiotherapy and Occupational Therapy interventions.
  • Monitor patient progress and contribute to therapy planning within agreed guidelines.

  • Support neuro-rehabilitation by implementing therapy programmes devised by the lead therapist
  • Manage your own patient caseload, assessing and treating patients, and monitoring their progress.
  • Assist in patient assessments, gathering relevant information, and adapting treatment plans under therapist guidance.
  • Work closely with the multidisciplinary team, contributing to patient care planning and discharge processes.
