Are you an experienced Nurse Assessor looking for a new challenge?
A nurse assessor at Cygnet healthcare holds a critical role in managing the patient journey from referral to admission.
The position requires high level clinical judgement and the ability to operate as a professional representative of the organisation.
Joining our team on a permanent, full time basis you will work 40 hours per week across 5 days.
This post is predominantly for the areas South of England, Greater London areas. Nurse assessors are required at times to travel to different areas within the UK to meet referral demand.
Your day-to-day…
- Travel to healthcare sites (NHS And Independent) across the UK
- Provide comprehensive assessments across mental health, learning disability, acquired brain injury, Forensic & social care/residential services.
- Complete an in-depth Pre admission report demonstrating extensive knowledge of the assessment information, including a 5P risk formulation.
- Assess patients referred to Cygnet Health Care that will present with varied presentations, risks and health.
- Represent Cygnet in a professional, positive manner when carrying out assessments
- Exercise clinical judgement to assess patients
- Liaise with Hospital Managers and Business Relationship Managers to ensure the patient is placed according to their need and risk.
- Maintain working relationships with Internal and external professionals.

An experienced Nurse with a valid NMC Pin Number, you’ll have…
- Experience in producing accurate high quality assessments and reports.
- Must have experience within secure services.
- Substantial experience working with service users presenting with mental health, learning disability, and neuropsychiatric needs.
- Excellent communication skills
- Ability to work to deadlines
- Experience in producing accurate assessments and high-quality reports with in-depth knowledge of the 5P formulation.
- The ability to work alone and as part of a team
- Knowledge of the Mental Health Act 1983 & the latest nursing practices
- Compassion, resilience and a dedication to empower and support service user independence
- A valid driver’s licence and use of a car.
- Based in the south of England / London
