Pay: £24.25-£27.98 depending on experience

Accountable to: Head of Clinical Workforce
Reportable to: Clinical Manager
Base: Newfield House, Vicarage Lane, Blackpool, FY4 4EW. However, depending on the service, travel may be required.

We are looking for band 6 practice nurses and non-medical prescribers to join the team on a bank basis, covering all the services within Inclusion Health. We are looking for individuals with a passion and enthusiasm to work with some of the most vulnerable and complex people within our community. The right candidates will be approachable and display excellent communication skills, compassion, tolerance, and a non-judgemental attitude.
Main duties of the job
Day to Day Duties to include, but not exhaustive:
- To provide proactive reviews and support for patients with CVD and / or COPD in line with clinical competence and local pathways
- To initiate prescribed medication and amend medication when required.
- Undertake holistic health assessments, identifying physical, mental and social needs
- Deliver health prevention and health promotion interventions including lifestyle advice (alcohol, smoking, diet and physical activity), weight management and wellbeing support, Opportunistic screening and vaccination advice
- Identify unmet needs and escalating health concerns, taking appropriate actions
- Deliver care within the mobile clinical unit adapting consultations to a community-based environment
- Support people who present to the mobile unit by providing assessment and advise where appropriate, signposting and referring to the most suitable service when out the scope of practice and offering reassurance, health information and ongoing support
- Promote self-management and empower patients to engage with health services
- Accurately record all activity, assessments and outcomes on EMIS using appropriate clinical coding and templates
- Maintain confidentiality and data protection standards
- Maintain Current NMC registration adhering to the NMC code of conduct
- Work autonomously withing scope of practice, escalating concerns appropriately
- Participate in supervision, appraisal and continuing professional development

Job description
Compass Medical Practice is a GP practice for those who are difficult to manage in mainstream practice. Patients are either consulted via telephone/ video link or face to face. When a face-to-face appointment is required, a risk assessment is undertaken to assess what support the clinician requires. Consultations are supported with the use of patient liaison officers and consultations are carried out in our mobile clinic van or in an agreed prebooked venue.

Compass Medical Practice also provides medical cover to temporary patients residing at Thornton House. These are short term patients that are residing at Thornton house for either rehab, rest or recouperation. There is a weekly ward round and multidisciplinary team meeting. Compass Medical practice also offers medical support throughout the week, outside of the ward round, during opening hours.
Our homeless health service is nurse led and is operates on a Monday, Tuesday, Thursday and Friday from the Salvation Army in central Blackpool. The nursing team take a multidisciplinary approach, often working with wider services to meet the needs of a complex patient cohort who are facing multiple disadvantage.
The Targeted demand management (TDM) service supports people across Blackpool to address clinical and non-clinical factors increasing their risk of an unplanned hospitalisation.
Key aims of TDM are to provide clinical and non-clinical interventions that meet the needs of individuals identified in Blackpool with the following respiratory or cardiovascular long-term conditions. The TDM service, through reducing individual risk, will play a role in relieving pressure on acute services, through preventing health crises.

Multidisciplinary Working
Work collaboratively with other Blackpool Based organisations such as Healthwatch, PCN Social Prescribers, Community Nursing, Housing Intervention teams.
Participate when required in multidisciplinary team for patients experiencing homelessness.
