Nurse Case Manager

Office Location: Fazakerley (minimum 2 days per week)

Area Covering: Merseyside and West Lancashire

Salary: Up to £40,000 depending on experience

Contract: Full-Time, Permanent

An exciting opportunity has arisen for a Nurse Case Manager to join Routes Healthcare, where you'll play a pivotal role in supporting and overseeing complex care packages — maintaining the highest standards of clinical excellence and facilitating truly person-centred care.

What your day-to-day will look like

  • Managing complex care packages, both clinically and holistically
  • Developing, reviewing and implementing care plans and risk assessments
  • Managing the clinical training needs of the care team, maintaining an appropriate skill mix to ensure safe and effective practice
  • Engaging in networking, attending MDT meetings and working with clinical commissioning groups
  • Conducting new and ongoing competency assessments with care staff
  • Carrying out care package reviews
  • Auditing client notes and medication records

What we're looking for

  • RN qualification (Adults) with a valid NMC pin
  • Experience in a high dependency or critical care setting (advantageous)
  • Experience in tracheostomy and ventilation management in a community setting (desirable)
  • Computer literate with working knowledge of Word, Excel and Outlook
  • A good understanding of CQC regulations
  • Excellent communication skills, both written and verbal
  • Someone passionate about putting people first
  • Flexible, autonomous and well-organised
  • Access to a car for work purposes
