The Role
- Manage a caseload of six bespoke complex care packages.
- Complete comprehensive clinical assessments and develop person-centred care plans.
- Provide clinical leadership and support to care teams, ensuring the highest standards of care.
- Work collaboratively with families, CHC teams, commissioners and community professionals.
- Monitor clinical governance, medication management and care quality across your caseload.

About You
- Registered Nurse (NMC).
- Experience supporting children within a community or complex care setting.
- Experience caring for clients with tracheostomies and mechanical ventilation.
- Full UK driving licence and access to your own vehicle.
