Nurse Manager, Wound Centers

Job Description

• Day-to-Day Management: Manage the wound care team in terms of workflow, staffing, and coordination of patient care according to patient acuity and volume.
• Quality and Safety Assurance: Ensure staff provides quality, safe care according to current standards of practice for Wound Care in the administration of procedures/treatments/medications. Assist the Director in assuring Joint Commission standards are adhered to.
• Continuous Education: Keep up to date with best practice standards in Wound Care and provide ongoing education to the team where needed.
• Collaborative Practice: Develop, mentor, and coach continual improvement in the collaborative practice between physicians, nurses, and ancillary staff, along with community partners.
• Metrics Analysis: Analyze and review program metrics using a working knowledge of the reporting capabilities in the EHR with a focus on wound healing metrics and coordination of patient care.
• Efficient Care: Provide efficient and effective care with the guidance of the physician during the clinic sessions, attend the clinics where scheduled, and be dependable.
• Operational Workflows: Round on a regular basis in each clinic to assure operational workflows are adhered to.
• External Coordination: Serve as the point person for outside entities such as home care agencies and DME vendors to assure smooth transitions of care and educational opportunities where needed.
• Patient/Family Teaching: Ensure staff effectively provides patient/family teaching.
• Documentation: Ensure staff documents according to entity/dept. guidelines. Be proficient with computer systems and assist with optimizing the HER as it relates to the clinic workflow. Work with the IT team to implement any changes.
• Age-Specific Needs: Address age-specific needs of school age, adolescents, adults, older adults.
• Management Responsibilities: Carry out Management responsibilities in accordance with the organization's policies and applicable laws.
• Staff Management: Assist Program Director with interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems, and keeping Program Director informed of all issues needing further review.
• Other Duties: Perform other related duties as assigned.
• Continuous Improvement: Challenge current working practices; identify process improvement opportunities and present recommendations and solutions to management. Engage and commit to the organization’s culture of continuous improvement by actively participating, supporting, and promoting CCHC Pillars of Excellence.
