Job Overview
NHS@Home is an innovative service, providing care for people who are acutely unwell in their own homes across Bristol, North Somerset, and South Gloucestershire. It offers an alternative to hospital and helps people return home from hospital more quickly too.
The service uses a mixture of face-to-face visits, phone and video calls, and remote monitoring through digital technology, to help people recover safely in the comfort of their own home.
NHS@Home has virtual pathways, covering a wide range of conditions including frailty, respiratory, heart failure and general surgical recovery. Our team provide a very wide range of acute healthcare treatments within the home, and monitor health and vital signs for deterioration.
The service is run in partnership between Sirona care & health and the Bristol Foundation Trust, Southmead and City Bristol Operating units

Working in our team you will learn a wide set of acute care skills in a community setting. You’ll also contribute to a multi-disciplinary approach, undertaking board rounds and working alongside consultants, advanced care practitioners, nurses and therapists to devise effective treatment plans that make a real difference to the lives of local people
Main duties of the job
- Work collaboratively with the NHS@Home team at Southmead site, City Bristol Site, and system partners including Sirona, providing acute care for patients with complex and specialist needs across Bristol, North Somerset, and South Gloucestershire.
- Assess patients' physical, psychological, and social needs in both hospital and community settings, while working alongside the multidisciplinary team to develop, implement, and evaluate individualised care plans and treatment programmes, with the support of the senior team.
- Deliver high-quality care in patients' homes using a broad range of clinical skills, including venepuncture, cannulation, and wound care, while communicating effectively with patients, families, and healthcare professionals and utilising digital technology to support safe, responsive, and efficient service delivery.

The post holder will work in collaboration with the NHS@Home team at Southmead hospital and BNSSG system partners at Sirona and UHBW. Hours of operation are 0700-20:30
It is essential you hold a valid full UK driving license and have access to an insured car with business insurance. Travel expenses will be paid.

Qualifications
Essential criteria
- Nursing Associate with current UK recognised live registration with NMC PIN
- GCSEs Maths, English, Science Grade C or above
- Foundation degree (FdSc) Nursing associate or equivalent
Desirable criteria
- Evidence of CPD
Work experience
Essential criteria
- Competent in a range of clinical skills including observations, venepuncture, cannulation, wound care
- Experience of working in teams as part of an MDT
- Able to escalate confidently and seek advice where needed
Desirable criteria
- Varied clinical background post qualification inc community setting
Knowledge/skills/abilities
Essential criteria
- Understanding of the importance of following procedures and treatment plans
- Ability to organise and prioritise own delegated workload
- Ability to communicate effectively (written, verbal and non-verbal communication) with patients/relatives and carers and all members of the multidisciplinary team
- Ability to work independently within scope of practice
- Excellent computer / IT skills and expectation to develop these in line with the needs of the service
Personal qualities/special circumstances
Essential criteria
- Has a full UK driving license and access to car with business insurance. Community travel requirement across Bristol, South Gloucestershire, and North Somerset.
- Willingness to undertake training required for the role
- Able to work and remain calm under pressure
- Ability to undertake shift work to meet service demands, covering a 7-day period, including bank holidays
