Role Overview
The post holder will work as a member of the Primary Care Nursing Team under the direction of the lead nurse.
Key Responsibilities
- Support Registered Nurses, General Practitioners, and other health care professionals in the assessment, treatment, and management of patients presenting in primary care settings.
- Work as part of the nursing team within standard operating procedures, with Registered Nurses and General Practitioners available for reference.
- Assist with specified delegated clinical tasks, including the provision and monitoring of Chronic Disease management, providing a caring, patient-focused service at all times.
- Carry out delegated clinical duties within HMP & YOI Parc.
- Undertake Phlebotomy clinics and dry blood spot testing.
- Perform physiological measurements such as blood pressure monitoring, pulse monitoring, waist circumference measurement, weight monitoring.
- Provide necessary Health Promotion advice to patients and carers, e.g., Smoking Cessation, Weight Management Support, Alcohol Consumption.
- Undertake Spirometry testing, and record results in patient information.
- Undertake ECG monitoring.
- Participate in the administration of immunizations under a Patient Specific Directive following a period of training, i.e., B12, influenza, and pneumococcal injections.
- Perform basic wound management under the supervision of a Registered Nurse following appropriate training and assessment.
- Prepare the clinical environment for the patient, ensuring the treatment area is clean, stocked, and tidy.
